Provided below are various forms that require completion prior to your visit. Please download the appropriate forms as directed by physician and/or staff by clicking on the name of the form. Please bring the completed forms with you to your scheduled visit. This will help expedite the registration process.

Our Physicians are affiliated with NewYork-Presbyterian/Lower Manhattan Hospital . In case of emergency, if the office is closed, please go to the Emergency Department of NewYork-Presbyterian/Lower Manhattan Hospital and ask for the Orthopaedic House Physician onged.
If you require a referral for your insurance plan, you are required to obtain the referral from your Primary Care Physician before you can be seen. Please coordinate this with your Primary Care Physician. Appointments will be rescheduled if required referrals are not received in the office by the time of your schedule visit.
We accept most insurance plans. Please check with your insurance plan to determine if the visit will be considered in-network or out-of-network. At the time of your visit, you will require your insurance card, driver’s license and appropriate referral. If your plan requires you to pay a co-payment, coinsurance, and/or deductible you must pay at the time of your visit. Payment will also be required for non-covered services. For your convenience, we accept cash, checks, Visa and MasterCard. If you have questions regarding your bill, please call our office during business hours.
Some of our Physicians in our practice may not participate in your insurance plan. You may be personally responsible for payment of your bill. If your insurance plan provides out-of-network benefits, then you may be responsible for any co-insurance or deductibles that are required by your plan, or any balances that remain on your account after applying the insurance proceeds which we receive from your plan.
If your injury is work related you must provide us with the name, address and telephone number of the workers’ compensation insurance carrier. You are also required to have the carrier case/ claim number and date of accident. We will verify this information prior to making an appointment. Upon verification, we will call you to schedule the appointment. If your claim is denied by the insurance carrier(even though we have verified it) you are responsible for payment of your bill.
Please call our office during office hours for prescriptions and refills. We will need to refer to your records in order to comply with your request. Your Physician may recommend a visit for re-evaluation and examination depending on when you were last seen.
